Please fill out the below form, print, and mail to the above address. Send a check to the above address, and made out to Jireh Bible Church, c/o CD Request. Once we receive your payment, processing of the CDs will start. Please allow 2-4 weeks for delivery. Contact the church to arrange for local pickup.
